Provider Education

Community Care is Looking for Providers Serving Dane County

Community Care will begin providing Family Care services in Dane County as early as May. This means members enrolled in Family Care, or new members enrolling in Family Care, will be able to choose Community Care as their Managed Care Organization.

Community Care is hosting a series of Provider Introduction/Interest Meeting to meet with local providers. Please see below for meeting details.

NEW Provider Application Training Sessions!

Community Care is now offering Provider Application Training Sessions. There will be one session held the third Monday of every even-numbered month at our headquarters location at 205 Bishops Way, Brookfield, WI 53005. Dates are listed below.

These trainings are highly recommended for any provider applying to be in Community Care's network. These trainings are required for any provider who's application has been denied and are looking to reapply.

Please note:  Community Care is currently NOT accepting applications for 3 or 4 bed licensed adult family homes or 1 or 2 bed certified corporate-operated homes in Kenosha, Milwaukee, or Racine Counties.  (updated 5/7/19)

Community Care periodically offers educational opportunities to providers. Our educational opportunities are available to CCI in-network providers only. To learn more or register for a workshop, click the title of the event you're interested in to download and print the registration document.

Screening for Dementia in Persons With an Intellectual Developmental Disability

Training Curriculum: Alzheimer's Disease and Related Dementias
The Health Resources and Services Administration (HRSA) has eleven new training modules:

  • The modules are designed to teach the primary care workforce about dementia caregiving, and also to help providers address caregiver needs.
  • Four modules are aimed at providers, and seven are aimed at family caregivers.
  • These modules are an addition to the Agency's Alzheimer's Disease and Related Dementia's Training Curriculum.
