  • New Medicare cards are coming! Medicare is mailing new cards between April 2018 and April 2019

    The new card has a Medicare Number unique to you, instead of your Social Security Number. Your new card will automatically come to you—just make sure Social Security has your most up-to-date address.

  • Education Opportunity - Integrative Psychiatry: Collaborative Care to Mental Health

    Inspired by the realization that conventional psychiatry has become a linear, medication-dependent, symptom-suppression based model, with little to offer the millions of people struggling with mental illness, Dr. Tummala turned to Integrative Psychiatry, fully embracing its patient-centered, collaborative care approach. Unlike conventional psychiatry, Integrative Psychiatry attempts to find the root cause of a person’s problems and work towards achieving overall mental health and balance. She embraces an innovative, mind-body-spirit approach that combines the science of modern medicine with the wisdom of ancient medicine to address the root causes of physiological symptoms.

    Kenneth MunsonCommunity Care is extremely proud to achieve a significant milestone, our 40th anniversary.

    From the moment we opened our doors in 1977, Community Care has put the individuals we serve first. The commitment to quality care for members has always been our number one priority. Our success is rooted in satisfying members and the ability to adapt to their changing needs.

    Community Care continues to strive for excellence for those we serve. We would like to thank our current and former members and employees. Without the members’ confidence in us and the employees’ commitment to the mission, we could not have grown to this magnitude and have the ability to serve more individuals. 

    Reflecting on the long history of Community Care, I’d like to acknowledge Kirby Shoaf who built this company from a small building on Prospect Avenue to one of the largest Managed Care Organizations  in Wisconsin. It is an honor to continue his legacy he left behind since his retirement.

    We will continue to provide innovative, flexible services and constantly evolve so we can meet our members’ changing needs.

    We want to extend our thanks and gratitude to everyone for being a part of our 40 years of service as we look forward to continue serving the community.
