
Community Care received a grant from the Wisconsin Board for People with Developmental Disabilities to create a comprehensive educational program for youth transitioning into the adult long-term care system. The educational program developed by Community Care focused on the individual’s option to manage some or all long-term services, also known as self-directed supports. Community Care created informative, easy-to-read educational tools that included guidebooks, a website (
www.iselfdirect.org) and training presentations. The guidebooks were distributed to school districts, county aging and disability resource centers, Community Care members and their families. Community Care conducted 12 training sessions throughout its 11-county service area in addition to training and deploying SDS advocates to promote self-directed supports throughout the region.
