  • Honoring Choices Wisconsin charter member selection, Wisconsin Medical Society

    Honoring Choices WisconsinCommunity Care, Inc. was chosen as a charter member of the Honoring Choices Wisconsin advance care pilot program to represent long-term care providers. Sponsored by the Wisconsin Medical Society, Honoring Choices Wisconsin chose Community Care for its “pioneering work … and dedication to improving advance care planning.” The program was developed to be a national model for promoting advance care planning education and utilization.

  • Self-Directed Supports education grant, Wisconsin Board for People with Developmental Disabilities

    Wisconsin BPDDCommunity Care received a grant from the Wisconsin Board for People with Developmental Disabilities to create a comprehensive educational program for youth transitioning into the adult long-term care system. The educational program developed by Community Care focused on the individual’s option to manage some or all long-term services, also known as self-directed supports. Community Care created informative, easy-to-read educational tools that included guidebooks, a website (www.iselfdirect.org) and training presentations. The guidebooks were distributed to school districts, county aging and disability resource centers, Community Care members and their families. Community Care conducted 12 training sessions throughout its 11-county service area in addition to training and deploying SDS advocates to promote self-directed supports throughout the region.

  • Healthiest Employer Award, Milwaukee Business Journal

    Healthiest EmployersCommunity Care’s continued commitment to and investment in its employees’ health and wellness was recognized by the Milwaukee Business Journal when it received the 2013 Healthiest Employers Award. A member of the Wellness Council of Wisconsin, Community Care collaborates with industry leaders to promote and implement innovative activities and philosophies in health and wellness.

  • Dazzling Implementation Award, Halogen Company

    Dazzling Implementation AwardCommunity Care was the recipient of the Halogen Company’s Dazzling Implementation Award for its utilization of the USucceed talent management system. USucceed provides the tools to help employees identify and nurture their talents and continually manage their careers to improve job satisfaction, productivity and retention rates.

  • Tau Award, Clement Manor and School Sisters of St. Francis

    AwardClement Manor retirement community and the School Sisters of St. Francis presented Community Care with the 2008 Tau Award for “demonstrat(ing) exceptional leadership in promoting the value of older adults with educational opportunities for education, wellness and spiritual growth.”

  • Community Leadership in End of Life Care, Froedtert Memorial Lutheran Hospital and the Medical College of Wisconsin

    Froedtert & Medical CollegeCommunity Care was the recipient of the 2003 Community Leadership Award in End of Life Care from Froedtert Memorial Lutheran Hospital and the Medical College of Wisconsin. Community Care’s member-centered end of life care program focuses on the needs and wishes of the organization’s members and their families and includes the services of a palliative care specialist and chaplain.
