Our Commitment to Quality

Community Care is dedicated to providing quality care to those we serve. Our member-centered programs strive to improve the quality of life for frail older adults and adults with physical, intellectual or developmental disabilities by helping them live as independently as possible for as long as possible within their communities.

A few of our quality highlights:

  • Our organization is staffed with professionals who provide support in areas such as long-term care, behavioral health, rehabilitation, geriatrics, infection control and wound care.
  • Our 24-hour call center ensures members can reach us any time, any day of any week.
  • We are participants in several regional and national initiatives, including Million Hearts, Partnership for Patients, and Honoring Choices Wisconsin (note: clicking these links will take you to a different website)

Community Care has been the recipient of numerous awards and much recognition, but it is our members’ stories and experiences that we believe best speak to our commitment to quality.

In addition to our internal quality reviews, our programs are regularly evaluated by state, regional and national agencies to ensure our members receive the quality care they deserve.

As a managed care leader in Wisconsin, we’re committed to providing innovative, flexible services and we’re constantly evolving to meet our members’ changing needs. Please view our 2019 Quality Work Plan to learn about our improvement projects.

Every year, our quality department completes a quality plan. This plan includes performance improvement projects that we work on with our members to improve quality of care. We are working on our plan for this year and we want to hear from you!

If there are projects that you feel would be beneficial, we would like you to tell us your ideas. Some examples of projects that we have worked on previously include falls and exercise, high risk medication reduction, Honoring Choices, and reducing hospital readmissions.
