Interested in becoming a Certified One-or Two-Bed Adult Family Home?

Become a difference maker. As a certified Adult Family Home operator, you’ll make it possible for someone in the community who needs a little extra help to live more independently and reap the benefits of residing in a home setting.

We provide member-centered care to seniors and adults with physical, intellectual, or developmental disabilities living in Wisconsin. Each Adult Family Home (AFH) in our network works closely with our care team and certification team to promote a healthy, independent lifestyle for our members, while also providing safety and protection.

Community Care follows the Standards for Certified 1-2 Bed Adult Family Homes (P-00638).

If Community Care does not intend to place a member in the home at time of certification request, we may deny the request to certify and keep interest, information, or application for possible future need.
