Eligibility

Family Care Requirements

To be eligible for Family Care, you must be:

• An adult with a physical, intellectual, or developmental disability or be age 65 or older
• A resident of one of the counties in our Family Care Service Area
• Financially eligible for Medicaid; and
• Functionally eligible as determined by the Wisconsin Adult Long-Term Care Functional Screen

To stay enrolled as a member of our program, you must:

• Remain a resident of a county in our service area
• Continue to meet financial and functional eligibility requirements
