Long-Term Care Services Now Available in Seven Additional Counties

BROOKFIELD, Wis. - Community Care is pleased to announce that its Family Care program has officially expanded into seven new counties: Adams, Columbia, Dodge, Green Lake, Jefferson, Marquette, and Waushara. The expansion allows Community Care to offer long-term care options into new areas for older adults and adults with disabilities, ensuring greater access to essential services that support independent living.  

The Wisconsin Department of Health Services recently expanded Family Care and Family Care Partnership availability in South Central Wisconsin, doubling the number of managed care organizations (MCOs) serving the region. With this expansion, nearly 8,000 Family Care members in the newly added counties now have increased choices for managing their long-term care. The MCO changes are the result of a multi-year effort to improve the quality and accessibility of the state's long-term care services.

“This expansion allows us to provide personalized, high-quality care that empowers individuals to live as independently as possible in their communities”, said Kenneth Munson , Community Care CEO.

Family Care is a Medicaid-managed long-term care program that provides a range of services, including personal care, home modifications, supportive home care, transportation, and therapy services. Community Care’s continued growth ensures that more Wisconsin residents receive the care they need to maintain their quality of life, while remaining as independent as possible.
