Partnership
Our Family Care Partnership Program (HMO SNP), referred to simply as Partnership, is a program available to individuals who qualify for Medicare and Medicaid, or Medicaid only. Partnership serves frail adults (65 and older) and adults with disabilities by providing comprehensive medical and long-term care services.
Community Care Partnership provides prescription drugs to members with Medicare. Partnership members who do not have Medicare receive prescription drugs through Medicaid. When you enroll in the program, you will work with your care team to assess your needs and coordinate all aspects of your care with the goal of helping you live as independently as possible in your community.
