Provider Directories
About the Provider Directories:
- Providers must be properly credentialed to appear in the directory.
- Medically necessary services are available twenty-four (24) hours a day, seven (7) days a week, as appropriate.
- Provider's hours of operation are no less than the hours offered to commercial members.
Selecting a Provider:
Members work with their care teams to select in-network providers. Our care teams must approve all services before members receive them.
Provider Network Change Notice:
The provider network may change at any time. You will receive notice when necessary. Request a printed copy of the provider directory by calling (toll free) 866-992-6600.
