Family Care Member Handbook
Welcome to Community Care's Family Care program.
Family Care is a Medicaid long-term care program that helps elders and adults with physical, developmental, or intellectual disabilities.
People in the program receive services to help them live in their own home whenever possible. Family Care is funded by state and federal tax dollars.
The Family Care Member Handbook provides information you need to:
- Understand the basics of Family Care.
- Become familiar with the services in the benefit package.
- Understand your rights and responsibilities as a Family Care member.
- File a grievance or appeal if you have a problem or concern.
