Welcome to Community Care's Family Care Partnership program (also known as Partnership).
Partnership is a Medicaid program that helps older adults and adults with physical, developmental, or intellectual disabilities. It mixes health and long-term support services in home and community settings.
The Partnership Member Handbook provides information you need to:
- Become familiar with the medical care and long-term care services in the benefit package.
- See which services Partnership covers.
- Know your rights and responsibilities.
- File a grievance or appeal if you have a problem or concern.

The Evidence of Coverage below provides information about how Community Care covers your care.
Other parts of this contract include your enrollment form, the List of Covered Drugs (Formulary), and any notices you receive from us about changes to your coverage or conditions that affect your coverage. These notices are sometimes called “riders” or “amendments.”
