Family Care
Family Care is a Medicaid long-term care program designed to serve older adults and adults with disabilities. The goal of Family Care is to help members live independently in their homes or their communities for as long as possible. Family Care members have access to a variety of long-term care services that help to support their individual needs.
Benefits
A Team of Health Care Professionals
Coordinate and develop a care plan with a team of professionals.
Your care team will consist of:
- Care Manager
- Nurse
The Family Care program provides services that meet member's long-term care outcomes. You and your care team will work together to develop a cost-effective care plan.
Long-Term Care Services
This includes paid services from our network of providers.
Services may include:
- Adult Day Care
- Counseling Services and Education
- Employment and Community Services
- Medical Supplies and Equipment (approved in your care plan)
- Rehabilitation Services
- Residential Services
- Support and Care Services
- Transportation
The program does not cover physician and medical services, hospitalizations, or pharmacy services. Your Medicaid or Medicare benefits or personal insurance will cover these. You could be financially responsible for care services received from out-of-network providers that were not approved.
Active Involvement
We encourage active involvement from you, your family, and authorized representatives in determining the services and support that best meets your needs.
Manage Your Own Services
You will decide how and from whom you receive your services.
