From 1977-2024
1977: Community Care was Founded
We started our mission to provide services that empower the elderly and adults with disabilities to maintain their lifestyles and take control of their healthcare.
1982: An Alternative to Nursing Homes
We developed Community Options, a program that helps individuals to stay in their homes and their community.
1985: Protecting the Elderly
The Elder Abuse Reporting Program was developed.
1990: First Organization to Administer Pace
We introduced the innovative PACE model.
1996: Partnering with Physicians
The Wisconsin Partnership Program is developed, allowing members to use their physician of choice.

2007-2010: Expand Throughout Wisconsin
Community Care expands to Kenosha, Racine, Ozaukee, Sheboygan, Waukesha, and Walworth.
2011: Over 10,000 Members Milestone
Enrollment surpasses 10,000 members.

2015: Starts Day Center Services
Community Care and the United Community Center (UCC) partner to begin offering day center services to our members.
2017: Celebrating 40 Years
We celebrate 40 years as a non-profit organization, delivering quality care and excellence!
2018-2021: We Keep Growing
PACE expands to Kenosha county, making a total of 4 counties offering the Community Care PACE Program.
2020: Dane County Expansion
Family Care expands to Dane county, making a total of 15 counties offering the Community Care Family Care Program.
