Eligibility

PACE Requirements

To be eligible for PACE, you must be:

• 55 Years or Older
• A Resident of One of the Following:
  • Kenosha
  • Milwaukee
  • Racine
  • Waukesha
• Functionally Eligible
  • Functional eligibility is determined by the Wisconsin Adult Long-Term Care Functional Screen
• Financially Eligible for:
  • Wisconsin Medicaid
  • Medicare (ability to pay monthly premium)
  • Medicaid and Medicare
• Able to Pay Out of Pocket
  • Individuals have the option to pay privately out of pocket
