Our History
1970s
The trend to integrate people into the community begins in the 1970’s. Prior to this, long-term care was provided primarily in institutions.
1977
Kirby Shoaf creates Community Care to pioneer new ways to serve people with long-term healthcare and social service needs.
Late 1970's
A Kellogg Foundation grant and federal waiver funds the original Community Care pilot program.
1982
Community Options, a program designed to help individuals live within the community as an   alternative to nursing home care, is developed.
1985
The Elder Abuse Reporting Program is created by Community Care.
1988
Major contributors, including the Jane & Lloyd Pettit, and Robert Wood Johnson Foundations, support development of the innovative PACE model.
1990
Community Care becomes one of the first organizations to administer programs using the Program of All Inclusive Care for the Elderly (PACE) model.
1993
As PACE enrollment grows, a second Adult Day Center & Clinic opens on Milwaukee’s north side.
1994
The National PACE Conference is hosted by Community Care.
1994
Community Care opens Prospect Place, which provides affiliated housing and 24-hour care.
1996
The Wisconsin Partnership Program - a variation of the PACE program, is introduced, giving members the option to use their community-based physician.
2003
The Palliative Care Program at Froedtert & The Medical College of Wisconsin honors Community Care with the Community Leadership Award in End of Life Care.
2006
Wisconsin awards a planning grant to Community Care to expand long-term care through the Family Care Program.
2007
Community Care opens Vliet Center to provide expanded member program services.
2007
Wisconsin Partnership Program expands to Kenosha County; Family Care is introduced in Kenosha, Racine, Ozaukee, Sheboygan, & Waukesha counties.
2008
The School Sisters of St. Francis bestow Tau Award recognizing “exceptional leadership in promoting the value of older adults.” 
2009
Family Care expands to Walworth County.
2009
PACE expands to Waukesha. 

New location includes Adult Day Center. 
2010
Family Care and Family Care Partnership expand to Calumet, Outagamie & Waupaca counties.
2011
Member enrollment surpasses 10,000 participants.
2013
Community Care, Inc. names Kenneth Munson Chief Executive Officer.
2013
“Healthiest Employer Award” from the Milwaukee Business Journal is received.
2014
Family Care expands to Fond du Lac, Winnebago, & Manitowoc counties, making a total of 14 counties offering the Community Care Family Care Program.
2015
Community Care earns the prestigious Wisconsin Forward Award, considered the premiere recognition of workplace excellence in Wisconsin.
2015
Community Care and the United Community Center (UCC) form a partnership as members in the PACE Program begin receiving day center services by UCC.
2017
Community Care celebrates 40 years of quality care and excellence! 
2018
PACE expands to Racine county, making a total of 3 counties offering the Community Care PACE Program.
2021
PACE expands to Kenosha county, making a total of 4 counties offering the Community Care PACE Program.
