 | Care Management Each Family Care member is supported by a care team of a nurse and a care manager. Together, you and your team will identify your needs and determine the appropriate services to meet them.

Services
The Family Care program provides services that meet member's long-term care outcomes. You and
your care team will work together to develop a cost-effective care plan. The plan will involve natural supports along with paid services from our extensive
provider network.
The program does not cover physician and medical services, hopstipitalizations or pharmacy service. Your Medicaid or Medicare benefits or personal insurance will cover these. For a detailed list of services within the Family Care benefit package,
download the Family Care Member Handbook. All services require an authorization prior to receiving them.
Services that you may receive include: Quality Assurance/Outcomes
You, your family and your care team work together to ensure the services you receive are coordinated and delivered. You will meet with your care team at a minimum of every six months to review your care plan and ensure the services you are receiving meet your needs and support your outcomes. Your team will update your care plan if your condition changes or whenever they determine that the service you are receiving does not meet your needs or support your outcomes.
