What is Partnership?

Community Care's Family Care Partnership Program (HMO SNP) ("Partnership") for frail elders and adults with physical, intellectual or developmental disabilities provides comprehensive medical and long-term care. Community Care Partnership also provides prescription drugs for members with Medicare. Partnership members who do not have Medicare receive drugs from Wisconsin Medicaid and should contact Wisconsin Medicaid Member services at  1-800-362-3002 for more information. The focus is to help you live as independently as possible in the community.

As a member of Partnership, you will have a team of professionals that work with you to coordinate all aspects of your care. Your team will assess your needs and develop a cost-effective care plan.




Partnership Highlights:

Partnership members have access to medical care and long-term care services without the worry of insurance bills, co-pays, deductibles or insurance gaps. Partnership members with Medicare also receive prescription drugs and certain over-the-counter medications from Community Care and do not have co-pays or deductibles for drugs. Partnership members who do not have Medicare receive prescription drugs and certain over-the-counter medications from Wisconsin Medicaid and may have co-pays for drugs. Care for all your needs. Our Partnership program is designed to provide fully-integrated care, including medical, residential and home and personal care services.

Join a team of professionals. Each Partnership program member works with a team including a nurse practitioner, care manager and registered nurse. You and your team will evaluate your needs, abilities, resources and preferences and develop your care plan. Your team will then coordinate and authorize the services that will help you achieve the outcomes identified in your care plan.

You play an active role in your plan. Partnership members, their families and authorized representatives are encouraged to take an active role in developing the care plan. We will work with you to determine your strengths, resources and which services and supports meet your needs and are cost-effective and make sure your care plan continues to work for you. 

You can manage your own services. As a member of the Partnership program, you can choose the Self-Directed Supports (SDS) option to manage some or all of your long-term care services. Choosing SDS means will have more say in how and from whom you receive services.
